Care Improvement Plus is a Medicare Advantage Special Needs Plan for beneficiaries with certain chronic illnesses, offering unique services that go beyond what Original Medicare and many traditional Medicare Advantage plans offer.
This new approach to healthcare for people living with diabetes, heart failure, chronic obstructive pulmonary disease and/or end stage renal disease is the result of the Medicare Modernization Act — a law passed by Congress, which made more specialized health plan options available for Medicare beneficiaries with chronic illnesses.
For Members
For members, the plan provides all of the benefits of Original Medicare with additional services, including:
- Dental, transportation, and vision coverage with most plan options
- Personal health assessments and support from specialty trained nurse care managers as necessary
- Medication monitoring and counseling services
- Health education services specific to individual health conditions
- Tools to help manage and monitor health status and detect early warning signs of potential complications.
