Care Improvement Plus is a unique Medicare Advantage Part D (MA-PD) health plan exclusively designed to meet the specific needs of Medicare beneficiaries living with diabetes, congestive heart failure (CHF), or end stage renal disease (ESRD)people whose health can be very dependent on the good health decisions they make. Care Improvement Plus offers more benefits than Original Medicare and many other Medicare Advantage plans, such as a Personal Support Team and in-home health monitoring.
In 2003, Congress passed the Medicare Modernization Act (MMA) and paved the way for the development of more specialized health plan options for Medicare beneficiaries. It was this legislation that sparked the creation of Care Improvement Plus.
For Patients
For patients, the plan supplements current Medicare coverage with new services, including:
A Personal Support Team. A local Support Team consists of Care Managers, Care Nurses, and Telephone Nurses, working closely with the patients physicians and caregiver. The services include:
- Preventative screenings
- Periodic assessments and visits
- Telephone follow-up calls
- Education and support services
In-home Health Monitoring. To identify problems before they become acute, the Support Team performs routine health evaluations in the patients home, using the latest, easy-to-use support tools for specific conditions.
Comprehensive Prescription Drug Coverage for Medicare Part D.
Because the Care Improvement Plus Support Team works with the patient and physician between office visits, medication plans can be developed that best suit patients changing needs. Medicare Part D coverage pays for the medications prescribed by patients physicians, with only modest co-payments.
