About XLHealthWhat We DoFor PatientsFor PhysiciansNewsroom

For Physicians

At XLHealth, we understand how demanding your schedule can be, especially when treating chronically ill patients. Today's complex healthcare system can put significant constraints on your time and your resources.

National Guidelines Used in XLHealth Disease Management Programs

XLHealth bases all interactions with patients and doctors on well-established, national clinical guidelines which include:

  • American Diabetes Association (ADA) Standards of Medical Care in Diabetes
  • Joint National Committee (JNC7) on Prevention, Detection, Evaluation, and Treatment of High Blood Pressure
  • National Cholesterol Education Program (NCEP) Expert Panel on Detection, Evaluation, and Treatment of High Blood Cholesterol in Adults
  • American Heart Association (AHA) AND American College of Cardiology (ACC) Guideline Update for the Diagnosis and Management of Chronic Heart Failure in the Adult

We've designed our services with these challenges in mind. While we do not treat patients or provide care, we deliver the day-to-day support you need to give your chronically ill patients the best care possible.

What if my office already provides these services?

Your busy schedule probably makes daily per-patient care a virtual impossibility. And as you are probably all too aware, most health systems seldom pay physicians like you to provide the day-to-day care and attention your chronically ill patients need.

XLHealth has the proprietary tools, state-of-the-art information systems, and devoted staff necessary to provide these support services effectively and efficiently, day in and day out.

Do patients like the programs?

Surveys show that more than 95% of all patients and physicians that have worked with us in the past are extremely pleased with our programs. In fact, almost all of our patients say they would recommend our services to a friend or relative with a chronic illness.

How does XLHealth get paid?

Our client base includes individual patients and physicians, physician groups, health plans, and state and local governments in need of healthcare support in their market. We are typically paid a fixed amount and then rewarded for achieving improvements in clinical outcomes, healthcare spending efficiency, and patient quality of life.

Do XLHealth's programs improve quality?

Absolutely. Time and time again, our experience has shown that our work leads to dramatic long-term improvements in adverse cardiac events, amputation rates, glycemic control, kidney damage, and other chronic illness complications.
